Question:

My son is 8 years old having club feet with hyper extended knee in right leg. To date 5 tenotomies have been done. But still not fully corrected. I seek your advice for the same.

Answer:

Thank you for your question and concern about your son. Clubfeet can be challenging to treat, and depending on how stiff your son’s foot was at the beginning of treatment may determine how difficult it will be to treat long term. Your son has had quite a number of tenotomies. He is likely developing scar tissue behind his ankle that is persistent and robust and recurring as he grows. Although I haven’t seen his foot directly, I would probably recommend a more aggressive release at his next surgery, including the joint capsules of his ankle and subtalar joints. Sometimes heel bones can be scarred down behind the ankle making simple tenotomies less effective. A good way to see this at your son’s age is with an xray.
